Please send the completed form to derwent.hill@sunderland.gov.uk.  We will confirm receipt of your booking form within 5 working days.  Please contact us if you do not hear from us.  We will confirm your booking as soon as possible after this.

Friends of Derwent Hill Bursary application form

Bursaries provide financial support for children & young people from the City of Sunderland who would otherwise be unable to afford to visit Derwent Hill.  Funds are limited, so applications should only be made where there is significant need & there is no other source of funding available.  A maximum of 3 applications will be accepted per course.  If in doubt about whether to apply, please phone Derwent Hill to discuss.  The normal maximum grant is 50% of the course fee.

Personal details and consent forms must be completed by all participants taking part in activities.  You can bring a completed form with you to your event, or you can complete a form on arrival. We do not need the forms in advance, but if you have any specific dietary or medical requirements, please let your event or group organiser know so that they can inform us and help us to prepare appropriately.
